Hawthorne Referral Services


Thank you for using Hawthorne Animal Hospital’s referral services.  We strongly believe that a close working relationship with a patient’s regular veterinarian offers the highest quality medical care for the pet.  After completion of all procedures, you will receive copies of all medical records regarding the emergency or referral services your patient received while at Hawthorne.  Some of the many services we offer for referral patients are:

Overnight Hospitalization
Intensive Care Monitoring

Preparing for Transfer


We ask that RDVMs call Hawthorne ahead of transfer so we can better prepare for your patient’s arrival.   To help make your patient’s transfer a smooth transition, we ask that the RDVM complete the RDVM Transfer Form and fax or send it with the patient.  We also ask that copies of all lab work, medical records and radiographs pertaining to the case be sent with patient or prior to transfer.  Our DVMs and staff will work with you to help provide a health care plan with treatment costs so your client can be aware of ensuing costs.  Patient records can be sent to:

During Hospitalization


During treatment, RDVMs will be kept informed of their patient’s status during daily morning rounds.  This can be completed via fax, phone, email or text (based on your request). Should the patient’s status/treatment change during the day, we will do our best to send additional documentation so you are aware of the updates.  Should you have further questions, you can contact our staff at any time and ask to speak to the veterinarian and/or technician assigned to the care of your patient that day.

Post Care Discharge


We strongly believe that a close working relationship with a patient’s regular veterinarian offers the highest quality medical care for the pet.  After completion of all procedures, you will receive copies of all medical records regarding the emergency or referral services your patient received while at Hawthorne.   Should the patient need follow up care, we will continue to provide letters of service to you after each visit.
